Most common mistake is normally not taking regular backups or scheduling a backup at a wrong time. For example, on 30th June is the financial year closing at most places; so, people often forget that their backups are scheduled at nights and mostly clients especially FSIs are working all night. So, if back up is scheduled, it would eat up all bandwidth and sometimes even won't let commit the last minute changes.
