How to set up two differents line profiles at the same time to deliver a public IPv4 address and a SIP service in ONT EG8145V5

We are actually using ONT EG8145V5 and currently we have a requirement to deliver four IPv4 public address and SIP service.

Actually we have set up a SIP service in a ONT and after that we log in with admin user of the device and set up an static public IPv4 address in a specified LAN port and after we made the configuration, we didn't have connection in that LAN port and also Wi-Fi 2.4Ghz was not able to connect to intenet.

Could you explain us how can be done this service delivery or if it is possible with the same ONT EG8145V5?
